City of Torment mentions at least 4 more elder evil infested stars (plus Hadar is mentioned in 5e PHB), although only Caiphon gets the spotlight. In that case, should I use the info provided in 4e Monster Manual 3 instead?

Not sure I understand the question. What is the discrepancy between the sources?

There is no discrepancy. But the info in MM2 and MM3 is mostly about PoL. The novel only gives the color of those stars and that's all.

The elder evil-stars are related to the star pact in 4e. The star pact was changed to the Old One patron in 5e. Should I add the stars to the Great Old One patrons category?
