I've been wanting to make this table (source: Jeff Grubb (April 1987). “Plane Speaking: Tuning in to the Outer Planes”. In Roger E. Moore ed. Dragon #120 (TSR, Inc.), pp. 42–43.) for a long time:

  1. Glass always breaks in casting of spell.
  2. Quartz breaks about half the time in casting of spell.
  3. Precise determination of destination layer not possible.
  4. Lower layers may be reached through random chords.

This is amazing. An excellent addition to plane shift lore. And pretty cool for musically inclined players and DMs! :)

Is there a more recent version of this table that includes newer planes such as the Feywild and the Shadowfell, for example?

If there is a newer version, I don't know if it. I myself was wondering how one might get to even the Plane of Shadow, to say nothing of the 4e or 5e planes. It is easy for a DM to fill in some missing notes or materials or fancy chords. Maybe adamantine B for Shadow and mithral B for the Feywild? I think I might have my players have to figure out that an augmented chord takes them to the fourth layer. :)
