I've been wanting to make this table (source: Jeff Grubb (April 1987). “Plane Speaking: Tuning in to the Outer Planes”. In Roger E. Moore ed. Dragon #120 (TSR, Inc.), pp. 42–43.) for a long time:

This is amazing. An excellent addition to plane shift lore. And pretty cool for musically inclined players and DMs! :)
Is there a more recent version of this table that includes newer planes such as the Feywild and the Shadowfell, for example?

If there is a newer version, I don't know if it. I myself was wondering how one might get to even the Plane of Shadow, to say nothing of the 4e or 5e planes. It is easy for a DM to fill in some missing notes or materials or fancy chords. Maybe adamantine B for Shadow and mithral B for the Feywild? I think I might have my players have to figure out that an augmented chord takes them to the fourth layer. :)
The Far Realm.[]
I have come up with a tuning fork for the far realm: Black quarts tuning fork tuned to a D4 minor, this opens a swirling Watercourse green portal.
